Home Health: We'll assess your current state of health and make sure that you're receiving the home health care that you need. Health Maintenance: We'll monitor your health for changes, and take action accordingly Skilled Nursing: If you need skilled nursing at home or at a facility, we can make recommendations and arrangements. Family Contact: We can act as a local "family member", providing up-to-date communication and acting on behalf of long distance family members. Coordinating with physician: We can act as an intermediary, working with your physician to make sure your medical needs are met. Special equipment: Can specially adapted home equipment and furniture aid your independence? We can determine and recommend products.
