The Needs Assessment is the single most important step in helping you secure the quality of life you want.

The Interview:
The process begins with an extensive, confidential interview. Together we'll:

• Determine your goals, priorities and concerns
• Identify social and daily living needs
• Analyze the status of your legal and financial arrangements
• Discuss improvements for your home safety
• Review current / future health care arrangements
• Discuss possible retirement living options, home health providers or adult day care options as appropriate

Analysis & Recommendations:
We prepare a detailed written or verbal report that assesses your situation in goal-oriented areas and provide specific recommendations.

The Assessment Review:
We may meet again to review your report and agree on priorities and selected options. Or we may create the plan at the time of the initial meeting.

Follow-Up:
We may work with you to implement, or to guide you in the implementation of the agreed upon plan. As needed we will communicate or meet to update the plan and make changes.
